Patient Paperwork:
Please arrive 15 minutes prior to your massage appointment time so you can relax & unwind.
If you will be receiving acupuncture, please fill out the Acupuncture (medical history) Intake Form below prior to your appointment and bring it in with you to your initial acupuncture treatment. The Massage Therapy and Sauna Forms are below as well, if you'd like to fill them out prior to your appointment. Please contact us if you have any questions about the forms or need them mailed to you.

Cancellation Policy:
We appreciate your business which is why we reserve your appointment time specifically for you. In the event that you need to cancel or reschedule your appointment, please notify us at least 24 hours in advance. Individuals that arrive 10 minutes past their scheduled appointment time are subject to a shortened treatment. Individuals arriving 15 minutes past their scheduled appointment time will be asked to reschedule. Individuals who fail to call or arrive at scheduled appointment time will be subject to paying in advance for future appointments.
Method of Payment:
Cash, Checks, and Credit Card (Visa, Master Card, Discover & American Express) are all acceptable forms of payment.
Insurance Coverage:
Payment is required at the time of service. Some services may be covered by your insurance plan. A super bill can be provided for acupuncture for you to submit for reimbursement from your insurance company. Please contact your insurance company directly to see if your treatment is covered for reimbursement.
Gift Certificates:
Gift Certificates for our services are available in any denomination for those special people in your life. Please call or contact us for purchasing information.
